We are excited for you to join us on our annual EFCA North Central District Blitz Conference in Duluth! There is some important information to read through before the registration, so please read below before filling out the form:
All students and Lifeguards must be at the church by 3:15PM to load the vehicles, and we will be departing at 4:00PM. Students are asked to bring money for Saturday Lunch and Dinner. LSM will serve meals for Friday Dinner and Sunday Lunch. There will be continental breakfast at the hotel.
All students attending Districts will be required to provide a medical release form from the conference. They will also be required to have a current LSM registration form on file. We will send a registration as part of the information packet if need be. Otherwise you can go to lakewoodfc.com/students and go to the Online Registrations page to complete one there.
If you or your student would like to use money from their Student Account to pay for some of the registration, please confirm with the LSM Office how much money is in their account. They can be reached at (218) 829-7251 or email@example.com. You can also connect with Jordan or Kellie at church.
To use a Student Account payment for registration, the payments must be made on the FIRST payment and done in increments of $10. (Because of how the sign up form works, Student Accounts money cannot be used for the second half of a payment.) In the discount code option enter in the following: 2019DBYA followed by the total dollar amount you want to use from the account. Some examples of the discount code can be found here:
